Description: Founded by community health centers in Washington State, Community Health Network of Washington (CHNW) is a community health center based delivery network whose mission is to improve the health status of our communities through the provision of high-quality, affordable, community-based health care to underserved individuals and families, and through sustaining the network of community health centers and affiliated businesses.
Community Health Plan provides managed care services for more than 225,000 individuals and families throughout Washington who are enrolled in Basic Health, Healthy Options, the State Children’s Health Insurance Program, General Assistance–Unemployable program and Medicare Advantage. The health plan’s delivery system includes over 300 primary care clinics, 1,600 providers, 8,000 specialists, and 90 hospitals. Community Health Plan features an incentive program that rewards its members for getting the preventive care they, and their family, need.
Community Health Plan covers Basic Health, Medicaid and CHIP individuals across the state. Community health centers are located in most counties across the state.
Work in child/youth health: As a health plan and network of community health centers, we have an interest in having all kids covered.
